How do I set up/change/cancel an appointment?
Call 513-860-5200 to speak to our receptionist during regular business hours. Please cancel appointments with at least 24 hour notice. This courtesy to your fellow patients will allow someone else to use that time.
How do I get to the office/hospital?
We are conveniently located in the Mercy Medical Arts building next to Mercy Fairfield Hospital.
Do I have to pay for my visit at the time of my appointment?
If you have insurance, your copay will be due at the time of the appointment. We will bill your insurance company for its portion of the charges. If you do not have insurance, the initial office visit fee will be due at the time of the visit?
How much is my co-pay?
Your copay is determined by your insurance company and is usually printed on your ID card.
How can I get a prescription refilled?
Please call the office for prescription refills. In general, refills are available only for chronic maintenance medications, and only if you are not due for a scheduled follow-up visit.
How can I find out results to my tests?
Usually, results of tests ordered by Dr. Chait are discussed at the follow-up visit.
What do I do in an emergency?
In the event of a life threatening emergency, immediately call 911 or go to the emergency room. For urgent, but non-life threatening conditions and problems, please call the office. After hours, you will be forwarded to Dr. Chait’s voice mail/paging system to leave a message and/or page the doctor.
Who should I contact if I get sick after office hours or on weekends?
You may call the office number to contact Dr. Chait.
When is the best time to call the office?
You will find it best to make routine calls during our regular office hours. Our staff are available to respond to your telephone calls and answer many of your questions. If they are not able to answer your question immediately, it will be forwarded to the doctor. His recommendations will be relayed to you by return phone call, as soon as possible. For urgent problems after office hours, please call 860-5200 to have Dr. Chait paged.
How does Dr. Chait determine his fees?
Charges for office visits vary depending on the level of service provided. This is determined by the severity and complexity of the problems managed. There will be additional charges for any diagnostic testing or therapeutic procedures. Our fees are comparable to those charged by other Otolaryngologists in this area.
Will you send me a bill or statement?
You will receive a statement each month, if your account has a balance due from you. Generally, you will be billed after your insurance company has paid their portion, or if there has been no response from the insurance company. |
